Please follow these steps to get registered for the 2019-20 Season:

1. Fill out the registration form Online or download a PDF.
2. Complete the Medical Release Form.
3. Parents and the player must sign a Code of Conduct Agreement.
4. Provide a copy of your medical insurance card.
5. Provide a copy of each player’s Sports Physical.
6. Payment – complete with the PayPal link below or write a check to Western Kentucky Trailblazers
